How to get a Facemask
There are a few easy steps that must be followed in order to obtain a custom facemask for yourself, your child, or your athlete.
Step 1: A prescription from your doctor
When the prescription is written it needs to have the name of the patient, the date that it was written, the specific diagnosis, and what is being prescribed. It is typical for the physician to write "Custom Facemask" or "Custom Protective Mask" on the prescription.
Step 2: Photos
In order to establish proper trimlines for a facemask, Jeremy will need to work from some simple headshots of the patient. He needs three (3) photos:Photos can either be emailed to MHSRC (preferred) or printed and included with the shipping package. Photos can be emailed to:
- Frontal, full face image
- Right facial profile
- Left facial profile

Step 3: A facial Impression
This step is the most difficult step to complete. In order to create a custom facemask we need to have an impression, or cast, of the patients face. This is what we use to make a positive working mold of your face to create a truly custom facemask. There are many ways to have an impression taken. An orthotist (which can be located in your area at www.oandpcare.org), treating physician, team trainer, or physical therapist are all options. We have even had parents, coaches, or friends take impressions with great success. Step 4: Payment
Please contact us to arrange payment for the facemask. If we participate with your insurance, our billing department will be happy to bill that first, although frequently this type of service is not covered.
If we are unsuccessful or do not participate with your insurance we can arrange payment to fit your current situation. We are happy to accept a credit card over the phone (Visa, MC, Amex, Discover), personal check (which can be mailed with the impression), or arrange a payment plan to fit your needs.
The total cost of your mask will vary depending on the specific mask required (yes, we make more than one style of mask!), shipping preference, and any other special considerations (such as same day or next day turn around). Please contact us to determine billing/payment prior to mailing in your impression.
Step 5: Sending it all in
When the impression is complete, please pack for shipping as though you were sending a fragile artifact. Bubble wrap is great, just be careful not to wrap too tightly and distort the impression. It is impossible to understate the importance of shipping your impression carefully. We have no way of knowing the correct dimensions of your impression if it arrives damaged or misshapen. The fit and function of the mask is certain to be affected if the impression does not arrive at our office in the same condition that it came off the patients face.
